What is Social Prescribing?

Many things affect our health and welbieng such as feeling stressed about work, money, or what is going on at home.

People may visit their GP because they may be feeling stressed, anxious, or low in mood or because they are feeling they are on their own. All these things can affect our physical and mental wellbeing.

But these problems cannot be fixed by medicine or doctors alone.

That is where social prescribing comes in.  Social prescribing connects people to practical and emotional support, through Social Prescribing Link Workers, who are based in GP Practices and work alongside the Doctors, Nurses, and Practice Staff.

Social Prescribing Link Workers have time to build a trusting relationship with you and to talk about what matters to you.

Together with your Social Prescribing Link Worker, you will work out a plan to introduce you to the people and support that you need.

Social Prescribing can help people get more control over their health and wellbeing and to manage their needs in a way that suits them. It can especially help people who:

  • have one or more long-term condition
  • need support with their mental health
  • are lonely or isolated
  • have complex social needs which affect their wellbeing.

Social Prescribing Link Workers are people:

  • Who listen
  • Who are practical and helpful
  • Who will not judge you

Access the Service

You can access the service by either of the following options

1) Contact your GP practice and ask to be referred to the social prescribers

2) (Over 18s only) Email hnyicb-nl.southpcn-referrals@nhs.net stating your: Registered GP Practice, Full Name, Date of Birth (dd/mm/yy), Preferred Contact Mobile/Email, and NHS Number (if known)
